Free and Reduced Lunch Application (English or Spanish)
The free and reduced application form is available annually in mid-July.
Permission Form to Share Information with Other Programs: This form enables the FSSD to share information which you provided on your Free and Reduced Price School Meals Application with other programs for which your child(ren) may qualify.
Non-Release of Directory Information (English or Spanish): Please note that this form precludes your child’s photograph and information from being included in all school (e.g.: the school directory, newsletter, school programs and the yearbook). The form remains active and follows your child throughout their time in the FSSD. It may only be voided by parent or guardian signature.
